How to find a provider (cont.)

All documents must be submitted before Self-Arranged Child Care Providers are determined eligible.

Your Self-Arranged Child Care Provider is assigned an account representative.  If your provider has not received a Service Delivery Summary (Form 2455-A) by the end of the month, and your service has already been approved, please contact your account representative. Self-Arranged child care providers use Form 2455-A to monitor, document, and submit attendance of a child enrolled in Workforce Solutions for the Heart of Texas’ child care program.  This form can be found on our downloads page.  For more information, you can visit your local Workforce Solutions Center or call a Child Care Services representative at (800) 772-2269 or (254) 296-5361.
