Choosing a Relative to Care for Your Child

Parents may choose a licensed or registered child care provider or a relative. Family members that may be eligible to provide care for your child will be at least 18 years old and the child’s grandparent, great grandparent, aunt/uncle, or sibling.

In addition, Relative Care Providers must meet the requirements of DFPS for becoming a LISTED FAMILY HOME. There are no minimum standard rules for listed homes. The materials your relative will need to provide to Child Care Licensing to become listed include:

  1. A Listing Request form, Form 2986;
  2. A Request for Criminal History and Central Registry Check form on all applicable persons, Form 2971; and
  3. A Child Care Fee Schedule form, Form 2988, and
  4. Pay a $20 application fee when you submit the application materials to Licensing.

To download the forms, visit the Department of Family and Protective Services.
